It sounds like you take pretty good care of your mouth, and if your breath still smells, you may want to consult a doctor. I forget the name of the condition(s), but even the best mouth care can’t prevent certain types of bad breath.

Brush your tougue with a tooth brush and scrape it with a tougue scraper. Use mouth wash.
They sell tougue scrapers at the drugstore.
