Why do I get occassional bad breath?

I brush my teeth, floss, and use mouthwash daily. I went to the dentist a week ago and she told me that my gums and teeth were healthy. I also brush the tongue whenever I brush. I thought I might have something wrong with my inner organs, so I went to the doctor, but he too, told me that they were healthy. I do smoke, but I only smoke 1-2 cigarettes a day or a few per week. I also chew sugarless gum and drink water. Why do I have occassional bad breath? Sometimes I do and other times, I don’t. Why?

I have this problem that when I skip a meal or something, my breath really starts to smell (I think its called “hunger odor” or something). Otherwise, I smell fine, but if I skip one meal then I smell really bad. Gum or mints doesn’t work on it. I don’t skip meals often, but I need to occasionally, and I feel embarassed that i’m the only person who can’t skip a meal once in awhile. Does anyone know any solution to stopping hunger odor other than eating (drinking water doesn’t help me emuch either)? Also, I actually have no idea what I smell like when I am hungry (I just know that I don’t smell great), so can anyone describe to me what the smell is like so that I can sesne it better? I have a friend he is 16 years old.
Hes breath smells extremly bad especialy when we are in a tight place like bus or something hes smell go like a few meters even other people feel it.
He knows he has bad breath and tryed everything to cure it:
1. by himself at home cleaning 3 times+floss+tongue +water with salt for tongue + on bus parsley/gum(Orbit/Winterfresh/and others) EXTREMLY MUCH WATER he drinked water like each 30 seconds in bus
2.He got to dentist no luck….
I really want to help him as hes my best friend.
Also I don’t know if it matters but hes breath smells very bad when he breaths on nose He don’t need to talk to feel hes bad breath he just need to breath…..
So please any suggestions??
at least some mask for time on buss and stuff as hes very emberassed and avoid talking to me cause he go mo further from me and dont speak to me on transportation as it says I don’t want people to think it’s from U + I don’t want to make U suffer…
