Make sure he’s cleaning his tongue and gargling with mouth wash. Food particles get stuck on your tongue and in the back of your mouth and brushing and flossing does not help with this. Good luck!
Yes do what the first answer said also take him to a dentist because he may have dental problems that you are not aware of. Does he have any problems with sinus drainage? This can also cause bad breath. Have him checked. Good Luck.
